VISIT ONE
Tell Me What's Been Going On
We'll take time to review your symptoms, medical history, medications, risk factors, concerns, and goals.
When appropriate, laboratory testing, imaging, or preventive screening may be ordered to help complete the picture.
BETWEEN VISITS
Your Information Is Reviewed
Your history, available results, risk factors, and concerns are organized - so we can focus on what matters most during your follow-up.
VISIT TWO
Leave With a Plan
We'll review your findings together and discuss your personalized next steps.
You'll also receive your written Health Report Card and Health Roadmap - so you have a clear record of what we discussed and where your care goes next.
Your care should reflect your actual medical history, priorities, risk factors, and goals.
Depending on your needs, your personalized plan may include discussion of:
Preventive Care
We'll review age-appropriate screenings, immunizations, risk factors, family history, and preventive care needs so important items don't get overlooked.
Laboratory Evaluation
When clinically appropriate, laboratory testing may be ordered to evaluate or monitor conditions such as diabetes, cholesterol, thyroid disease, anemia, nutritional concerns, kidney function, liver health, and other medical concerns.
Medication Management
Current medications can be reviewed for effectiveness, side effects, interactions, monitoring needs, and whether changes may be appropriate.
Chronic Condition Management
Ongoing care may include conditions such as high blood pressure, high cholesterol, diabetes, prediabetes, thyroid disease, metabolic concerns, and other chronic conditions that are appropriate for virtual management.
Women's Health & Midlife Care
Primary care can also include discussion of menstrual changes, perimenopause, menopause, bone health, metabolic changes, and other health concerns that may become more important in midlife.
Coordinating The Bigger Picture

Common Questions
Can I Use This As My Primary Care Practice?
Yes.
The Root Care Family Health NP provides ongoing virtual primary care for adults in New York who choose the Primary Care membership after completing the initial consultation.
Is This The Same As Urgent Care?
No.
This is relationship-based primary care designed to understand your medical history, monitor your health over time, manage appropriate chronic conditions, and address preventive needs.
Some minor acute concerns may be appropriate for virtual evaluation, but this service is not a replacement for emergency care or situations requiring an in-person examination.
Do I Need To Have A Medical Problem To Schedule?
No.
You may schedule because you have a specific concern, need help managing an existing condition, are overdue for preventive care, or simply want to establish primary care and better understand your current health.
Can You Order Labs?
Yes.
Laboratory testing may be ordered when clinically appropriate based on your history, symptoms, diagnoses, medications, and preventive care needs.
Can You Refill My Medications?
Medication management can be discussed after your medical history, current medications, diagnoses, monitoring needs, and other relevant information have been reviewed.
Scheduling an appointment does not guarantee that a particular medication will be prescribed or continued.
Can You Manage Chronic Conditions?
Many common chronic conditions can be managed virtually when clinically appropriate.
This may include conditions such as high blood pressure, high cholesterol, diabetes, prediabetes, thyroid disease, and certain metabolic concerns.
Some conditions or symptoms may require in-person evaluation or specialty care.
What If I Need Imaging Or A Specialist?
When clinically appropriate, imaging, screening studies, or referrals may be recommended as part of your care.
Can I Still Use My Insurance?
The Root Care Family Health NP is a cash-pay practice.
Your insurance may still be used for covered prescriptions, laboratory testing, imaging, and other outside services.
Superbills are available for potential out-of-network reimbursement.
Will I Actually Meet With A Clinician?
Yes.
All visits are with Tara, a New York-licensed, ANCC board-certified Family Nurse Practitioner.
