Tara Lorzing-Wilmoth RN, MSN, FNP-BC

I’m Tara, a Family Nurse Practitioner and the founder of The Root Care Family Health NP.

After more than 17 years in healthcare, I created The Root Care Family Health NP to offer a different kind of experience—one where appointments aren’t rushed, concerns aren’t brushed aside, and there is time to look at the bigger picture.

Through The Root Care Family Health NP, I provide personalized virtual care for adults throughout New York, with a focus on primary care, menopause and perimenopause, and metabolic health.

Personalized care starts with being heard

My Background

My healthcare career began as a registered nurse in 2009. Over the years, I’ve worked across direct patient care, care management, clinical review, leadership, and family medicine.

I later became a Family Nurse Practitioner, expanding my role from supporting patients through the healthcare system to diagnosing, treating, prescribing, ordering testing, and developing individualized care plans.

Those experiences shaped the way I practice today.

I believe good care requires more than treating a single symptom or reviewing one lab value. It means understanding your history, your concerns, your risk factors, your goals, and how all of those pieces fit together.

Why I Created The Root Care Family Health NP

Traditional medical visits can feel rushed.

You may leave with a prescription but little understanding of why you feel the way you do. You may have several concerns but only enough time to discuss one. Or you may be told that your results are “normal” even though you still don’t feel like yourself.

I wanted to create a practice with more room for conversation, education, prevention, and thoughtful follow-up.

At The Root Care Family Health NP, my goal is to help you understand what is happening, what we can do about it, and what comes next.

There is no one-size-fits-all approach.

Your care should reflect you.

Care That Looks at the Whole Picture

Depending on your needs, we may look at areas such as:

Primary Care
Preventive care, routine medical concerns, chronic condition management, medication management, screenings, laboratory testing, and ongoing primary care support.

Menopause & Perimenopause
Hot flashes, sleep changes, mood changes, weight changes, vaginal symptoms, libido concerns, brain fog, and other symptoms associated with hormonal transition.

Metabolic Care
Weight, insulin resistance, prediabetes, diabetes, cholesterol, blood pressure, thyroid concerns, metabolic risk factors, and medication management when appropriate.

The goal isn’t to order every test or prescribe more medication.

The goal is to determine what actually makes sense for you.

I Understand What It’s Like to Be the Patient, Too

My approach to care has also been shaped by my own experiences navigating complex health concerns.

I understand what it feels like to live with symptoms that affect your everyday life, to manage changing hormones, to work through weight and metabolic concerns, and to want answers that go beyond “everything looks fine.”

Those experiences don’t replace clinical training—but they have made me even more aware of how important it is for patients to feel heard, respected, and included in decisions about their care.

I never want you to feel like just another appointment on the schedule.
